H.O.M.E. B.A.S.E. STREET OUTREACH – DOWNTOWN LANSING Together we serve, we build, we thrive.

The H.O.M.E. B.A.S.E. Street Outreach Program provides essential services to persons experiencing homelessness on
the street or other places not meant for habitation. The program offers the service of trained, experienced staff on
site in areas frequented by homeless individuals and families. Program staff create a cooperative with area
businesses, identifying and meeting needs of those who are homeless, including food, water, clothing items,
assistance with shelter, employment, literacy, and housing.

The H.O.M.E. B.A.S.E. Street Outreach Team, consisting of two staff positions, provides the following:

  • Surveys on foot and by car the designated areas agreed upon by the community to identify and engage persons who may be homeless.
  • Encourages and navigates clients that are willing to/interested in going to shelter, rehab, Community Mental Health, and/or similar services.
  • Works with clients to identify and relocate to habitable locations that do not endanger themselves or impede residential and/or retail traffic.
  • Provides clients with resource cards, bus passes, personal needs, water, portable food items, as needed.
  • Assists clients with obtaining necessary identification documents.
  • Completes mental health screenings with clients in defining and obtaining goals.
  • Uses de-escalation best practices to resolve conflicts between and among street homeless clients or other housed or unhoused individuals.
  • Works with community resources, including local law enforcement/municipal services to stabilize clients.
  • Works with clients to develop positive self-regard and promote productive community engagement for long-term housing and community stability.
  • Reports on a monthly basis or more often, as agreed upon, to community partners/funders regardingprogress, challenges, client engagement goals, community engagement goals, and plans forward.
